I hereby consent to participate in a driving program offered by DrivingMBA Phoenix, L.L.C. an Arizona limited liability company (DMBA). I understand and acknowledge that motor vehicle driving involves an inherently dangerous activity and, by the execution below, confirms that DMBA has made no representations or warranties, express or implied, or otherwise given any assurance or guarantee that participation in the driving program offered by DMBA shall eliminate the possibility that I may be involved in a motor vehicle collision. I further waive, release, absolve, indemnify and agree to hold harmless DMBA, and all persons, firms or organizations affiliated or otherwise associated therewith, including, without limitation, the claims arising either directly or indirectly as a consequence of participating in the program. The claims hereby affirmatively waived include, by way of illustration and not limitation, claims relating to property damage or personal injury, irrespective of the cause. By the execution hereof, the undersigned represents and warrants to DMBA that the undersigned has full power and authority to execute this enrollment agreement.
